Explanation for Reviewers

Reviewers are asked to review the problem and intervention lists and comment in any way to agree, disagree, or clarify. We particularly need feedback on the accuracy of the mappings of the problems and intervention listed below.

In addition we would be interested in views regarding a number of nursing problems. In some cases the patient may have an identified health need that is not necessarily considered an actual or potential problem. For example, supporting a new mother during her post partum period is not undertaken to resolve a problem or necessarily prevent a particular problem but is required to promote health during this time. This has therefore been termed “effective health maintenance + post partum period”.

A number of other “problems” have been treated similarly and described as “effective” health behaviour rather than “at risk,” “impaired,” or “ineffective.”

Problems

• P030 Impairment/Abnormalities in Infant is mapped to (ICNP) Impaired Physiological Status + Infant
• P031 Looked after and accommodated child is mapped to (ICNP) Effective Child Development AND Risk for Impaired Child Development
• P032 Possible developmental delay is mapped to (ICNP) Risk for Impaired Child Development
• P033 Child development support and health promotion requirement is mapped to (ICNP) Effective Child Development
• P033A Impaired Child Development is mapped to (ICNP) Impaired Child Development
• P153 Vulnerable Child is mapped to (ICNP) Risk for Child Abuse. Is this mapping accurate? Also note that P090 Possible child abuse is mapped to (ICNP) Risk for Child Abuse. Do we want to keep both?
• P021 Problem with Breasts is mapped to (ICNP) Breast Engorgement and Impaired Lactation. Are there other common types of problems with breasts during pregnancy and post natal period which should be included?
• P126-130 These are common tasks which may be carried out by, for example, a treatment room nurse who has little opportunity to assess and ‘diagnose’ the patient’s nursing problems. These items have been included in the problem list because they support the identification of disease – is this appropriate, should we keep these items or not?

Intervention

• A170 Children’s Hearing System is mapped to (ICNP) Collaborating with Legal Service About Child. Is this mapping accurate?
